To complete the sign-up process, you will be required to provide the following information:

- First and Last Name
- Date of Birth
- Full Street Address
- Last 4 digits of Social Security Number
Users must reside in a state where Slot Spot operates to successfully complete the sign-up process. Please refer to our terms and conditions for a full list of non-qualifying states
